You have little things to decide like interior seams, french seams, princess seam, cross-grain or not, panels/backing, gathers, smocking, hems, ribs, tucks, stitches etc... Depending on how complicated and the kind of fabric you wanted, you could spend anywhere from several hundred to a few thousand $. This is called having something custom made and fit.
My suggestion, if you want to go cheaper, find something that you like as far as basic design, fit and fabric. Then take it to a tailor/dress-maker and talk about what sorts of additions, changes you could make. Be aware that even doing this, certain types of fabrics or cuts (on the dress) may make it very difficult or impossible to make alterations to the dress other than taking it apart, or would be so expensive you might as well have something custom made.
